Osteochondrodysplasia (Scottish Fold Disease)

An inherited cartilage and bone disorder inseparable from the folded ear of the Scottish Fold — the same gene that folds the ear deforms cartilage throughout the skeleton.

The Scottish Fold's folded ear — the trait that makes the breed instantly recognisable and enormously popular on social media — is a cartilage defect. That is not an editorial framing; it is the biological mechanism. A dominant mutation in the TRPV4 gene weakens the cartilage of the ear so that it collapses forward. The difficulty is that the gene is not expressed only in the ear.

Cartilage forms the growth plates of every long bone and the smooth bearing surface of every joint. In a cat carrying the fold mutation, that cartilage develops abnormally throughout the skeleton, producing a condition called osteochondrodysplasia. Bone forms in irregular, lumpy deposits around the joints of the ankles, wrists and tail vertebrae; joint surfaces become uneven; and secondary arthritis follows. The tail is often the earliest giveaway — short, thick and unable to flex, so that running a hand down it feels like running a hand down a length of dowel rather than a chain of small mobile bones.

Severity depends on dosage. A kitten that inherits the mutation from both parents — the outcome of mating two folded cats — can be visibly crippled before it is six months old, with grossly swollen limbs and a gait so painful that euthanasia is sometimes the kindest option. Breeders responded to this decades ago by mating folds only to straight-eared cats, which avoids the catastrophic homozygous form. But research using radiographs, most notably work from Australia and later studies in Europe, has repeatedly found skeletal changes in heterozygous cats too. The consensus among veterinary radiologists is that there is no such thing as a Scottish Fold with entirely normal joints; there are only folds whose disease has not yet been looked for or has not yet become obvious.

For the owner of an affected cat, the practical task is recognising pain. Cats do not limp dramatically or cry; they simply stop doing things. They take the stairs instead of the leap, sleep on the floor instead of the windowsill, become less tolerant of being picked up, and are described as having "always been a quiet cat". Modern feline pain management is much better than it was — the anti-NGF monoclonal antibody injections in particular have transformed chronic joint pain treatment in cats — and many owners are astonished by how much personality returns once analgesia is optimised.

The wider question is one of breeding ethics rather than medicine, and the veterinary profession has been increasingly direct about it. A trait that is inseparable from a painful skeletal disease cannot be selected against while still selecting for the trait, because they are the same gene.

🔍 Causes

The condition is caused by a dominant mutation in the TRPV4 gene. That single mutation produces the folded ear that defines the Scottish Fold breed by weakening ear cartilage — and because cartilage is cartilage wherever it occurs, the same gene disrupts the growth plates and joint surfaces of the limbs and tail. Cats carrying two copies (homozygotes, from fold-to-fold matings) develop severe, crippling disease early in life. Cats with one copy — every folded-ear Scottish Fold — develop a milder but still progressive form, and radiographic changes can be found in essentially all of them if looked for.

🛡 Prevention

The only genuine prevention is not to breed folded-ear cats. Veterinary bodies in several countries, including the British Veterinary Association and the veterinary associations of Scotland and the Netherlands, have called for an end to Scottish Fold breeding on welfare grounds, and the breed is not recognised by the UK's Governing Council of the Cat Fancy. Prospective owners who want the look should consider a straight-eared Scottish Straight, which does not carry the mutation. If you already have a Scottish Fold, have it neutered and start joint monitoring early.

💊 Treatment

There is no cure and no way to reverse the abnormal bone growth. Management is pain relief for life: NSAIDs licensed for cats such as meloxicam or robenacoxib, the anti-nerve-growth-factor injection frotenamab or its equivalents where available, gabapentin for neuropathic pain, and joint supplements. Weight control is critical, as is a home adapted with steps, low-sided litter trays and soft bedding. Radiation therapy of the worst-affected joints has been used palliatively in some cases. Surgical fusion or removal of severely affected tail vertebrae is occasionally performed. Affected cats should never be bred from.

When to see a vet: Have any Scottish Fold assessed by a vet as a baseline, ideally with radiographs of the hocks, wrists and tail, even if it seems comfortable. Book an appointment promptly if the cat becomes reluctant to jump, changes how it uses a litter tray, sits oddly, or resents being touched along the back or tail — cats are extremely good at concealing chronic pain, and behaviour change is often the only sign.
