Portosystemic Shunt (Liver Shunt)

An abnormal blood vessel that bypasses the liver, allowing toxins to build up in the bloodstream — most often a congenital defect seen in young cats.

A portosystemic shunt is an abnormal blood vessel that connects the portal circulation (blood returning from the gut) directly to the general circulation, bypassing the liver. Normally, everything absorbed from the intestines passes first through the liver, which detoxifies ammonia and other by-products of digestion. When a shunt is present, those toxins flood into the bloodstream unfiltered, and the liver itself, deprived of its normal blood supply and growth factors, stays abnormally small and underdeveloped.

The great majority of feline shunts are congenital and consist of a single abnormal vessel. Signs usually appear in kittenhood: poor growth, a dull or "spaced-out" demeanour, drooling, and neurological disturbances such as circling, head-pressing, seizures, or temporary blindness. Because these signs stem from toxins reaching the brain — a state called hepatic encephalopathy — they are often noticeably worse in the hours after a protein-rich meal. A striking clue in some affected cats is a distinctive copper or gold colour to the irises.

Diagnosis typically begins with a bile acids blood test, which is highly sensitive for abnormal liver blood flow, followed by imaging — ultrasound, CT angiography, or scintigraphy — to locate the shunting vessel. Blood ammonia levels are also frequently elevated.

Medical management with a controlled-protein diet, lactulose, and antibiotics can control symptoms and is essential for stabilising a cat before any operation. The definitive fix, however, is surgical: gradually closing the shunt (often with a special ring device that narrows it over weeks) redirects blood back through the liver, which can then grow and function normally. Outcomes after successful surgery are generally very good, and many cats go on to live entirely normal lives.

Most feline shunts are congenital — a single abnormal vessel that forms before birth and lets blood from the intestines bypass the liver, so ammonia and other toxins are never filtered out. Purebred cats, particularly Persians and some Asian breeds, appear over-represented. Less commonly, shunts are acquired secondary to severe liver disease.

Because congenital shunts are inherited developmental defects, they cannot be prevented, though affected cats and their parents should not be bred from. Early recognition is what matters most: kittens that fail to grow, act dazed after meals, or have unexplained neurological signs should be tested promptly so treatment can begin before permanent harm occurs.

💊 Treatment

Definitive treatment is surgical closure of the abnormal vessel, which carries an excellent long-term prognosis in suitable cases. Before and instead of surgery, cats are stabilised medically with a protein-controlled diet, lactulose to reduce toxin absorption from the gut, and antibiotics to lower toxin-producing gut bacteria.

When to see a vet: Seek veterinary care for any kitten that is not growing normally, becomes dull, wobbly, or disoriented — especially after eating — or has seizures. A bile acids blood test is a simple, sensitive screening tool, and prompt diagnosis dramatically improves the outlook.
